What is the SDLMI-Transition Program?
Studies show that the SDLMI programs can:
- help students feel more in control of their choices,
- improve the way they make decisions,
- increase their involvement with neighborhoods and communities, and
- improve students’ abilities to get jobs.
The Self-Determined Learning Model of Instruction (SDLMI) is a research-based program that supports students with Individualized Education Programs (IEPs) in identifying goals, developing plans, and acting on those plans to achieve success. The SDLMI–Transition program has three phases that happen throughout 18 months. During this time, a trained instructor supports students to do the following:
- Set goals that help build skills to be successful in school and life,
- Develop action plans related to those goals, and
- Reflect on their progress toward the goals and adjust goals or action plans when needed.
Instructors talk with students about different topics that are related to students’ goals, such as better study skills, getting involved in clubs, identifying a trade, searching for jobs, and helping their communities. Instructors work with families and IEP teams to identify ways to best help students with their goals and action plans.
What is the SDLMI-Transition with Mentoring Program?
The SDLMI-Transition with Mentoring program offers students regular one-on-one mentoring during the school year and check-ins in the summer. During these mentoring sessions, instructors will work closely with students to set goals for after high school, like enrolling in college and getting a job. Instructors will also help students plan for the steps needed to reach their goals, monitor their progress on those steps, and identify resources at home, at school, and in their community to reach their goals. Instructors will also work closely with families to create a summer plan to help students stay on track.
